Authorization for Cremation Process as Final Disposition

I, the undersigned, certify, warrant and represent that I have full legal right and authority to authorize cremation and disposition for the above pet. I, the undersigned, request and authorize Precious Pets Cremation Services to take possession and arrange cremation of the remains of the above named pet.

The undersigned understands that ashes need to be picked up within 60 days of the date signed on the cremation authorization or ashes will be disposed of.

When choosing cremation as a method of final disposition for your pet, it is important that the pet owner understands that cremation is a final process and cannot be "undone". The undersigned agrees to indemnify and hold harmless Precious Pets Cremation Services and it's employees and agents from any liability or claims resulting from this authorization.

What happens after you send this form

Your form comes straight to our family. Precious Pets is family owned and operated, and we have served North Texas pet owners for four generations. Here is how it goes.

  1. We call you to go over your answers, confirm the details, and handle any questions you have.
  2. Together we settle the choices: private (individual) cremation or communal cremation, plus any keepsakes you would like.
  3. We arrange to pick up your pet, or we set a time for you to bring them to us, whichever you prefer.
  4. Ashes are returned home in about four days.

Nothing is final until we have talked. If a question has you stuck, skip it or take your best guess. We will sort it out on the phone.

What to have ready before you start

You do not need much. It helps to have a few things in mind.

  • Your name and the best phone number to reach you.
  • Your pet's name and a few basic details about them.
  • Where your pet is right now.
  • The type of cremation you are leaning toward. With a private (individual) cremation, your pet is cremated alone and the ashes come back to you. If you are not sure which type fits, we will talk it through when we call.
  • Whether you would like a keepsake. Urns, clay paw prints and other keepsakes are available.
  • A quick look at the pricing page, if you would like to see costs before you fill anything in.

Do not let a blank field stop you. Send what you know, and we will fill the gaps together.

Pickup or bring them to us

Both options are open to you, and neither one is required.

If you would like to bring your pet to us, we are at 1111 Haltom Road, Suite B in Fort Worth. The lobby is open Monday through Friday, 8am to 5pm, and you are welcome to come in and see the facility.

If you want us to come to you, we pick up across Fort Worth and the surrounding cities, including Arlington, Keller, Hurst, Bedford, Euless, Irving, Grapevine, Southlake, Colleyville, Mansfield, Burleson, Benbrook, Grand Prairie and North Richland Hills. Tell us where your pet is, and we will work out the rest with you.

Some families want to make the drive themselves. Others prefer not to. There is no wrong answer here, so pick whichever is easiest on you.

Paying for it

Every price is published on our pricing page, so you can see the full cost before you decide anything. We keep our prices out in the open on purpose. You do not have to ask for a quote to know what things cost.

We also accept CareCredit financing if paying over time would help. If you already have a CareCredit card, you can use it here. And if anything about cost is unclear, ask us when we talk. We would rather you know the numbers up front than worry about them later.

If you would rather just call

Some people would rather talk than type, especially on a day like this. Call (817) 862-7000 and we will take the details over the phone and set everything up with you directly. The phone is answered 24 hours a day, seven days a week. If it is 2am and you do not know where to start, that is exactly what this line is for.

If your pet is at a veterinary clinic, or you just want to hear how this works before deciding anything, start with a call. A call does not commit you to anything. And if talking face to face is easier, come by during lobby hours. However you reach us, we will handle the rest.
